By submitting this form, I acknowledge and agree to the following: Voluntary Participation I am volunteering my time and services to K9 Sentinels (K9SOH) of my own free will. I understand that no compensation, insurance, or other benefits are provided to me. Assumption of Risk I understand that working with or around retired service dogs may involve certain risks, including but not limited to: bites, scratches, allergic reactions, or injuries from handling equipment. I accept full responsibility for any risks, injuries, or damages that may occur during my participation. Release of Liability I release and hold harmless K9 Sentinels (K9SOH), its staff, volunteers, and affiliates from any claims, demands, or liability of any kind arising from my volunteer activities, except in cases of gross negligence or willful misconduct. Medical Treatment In the event of injury, I authorize K9 Sentinels (K9SOH) to secure necessary medical treatment at my own expense. I understand that K9 Sentinels (K9SOH) does not provide medical insurance coverage for volunteers. Photography & Media Release I grant permission for K9 Sentinels (K9SOH) to use my image, likeness, or voice in photos, videos, or other media for promotional purposes, without compensation. Code of Conduct I agree to follow all safety guidelines, instructions, and policies provided by K9 Sentinels (K9SOH) staff. I understand that failure to comply may result in termination of my volunteer participation.
